If you were certified with us last year and are due for a renewal of your medical card, you can renew with us at a discounted rate. If you are certifying for the first time or were certified by a different provider in the past, please select the service option Medical Cannabis Certification.

Cancellation Policy

Any cancellations made within 24 hrs of your appointment are subject to a $25 re-scheduling fee.
